The Care Coordination Assistant plays a critical role at Point of The Pines Gardens Assisted Living. This role is responsible for assisting with the coordination of care for residents within the facility. This position plays a pivotal role in ensuring the well-being and satisfaction of residents while maintaining a high standard of care and support in a resident-centered fashion.
Responsibilities:
1. Care Coordination: Assists with oversight and care coordination of the delivery of care services to residents. This involves assessing resident needs, developing personalized care plans, and monitoring their implementation.
Collaborate with healthcare professionals, including physicians, nurses, and therapists, to develop and implement personalized care plans that meet the unique needs of each resident.
2. Resident Assessment: Assists with conducting comprehensive assessments of residents' physical, emotional, and social needs. Work closely with residents, their families, and healthcare professionals to identify necessary services, therapies, and resources.
3. Care Plan Development: Assists with developing individualized care plans based on resident assessments, incorporating medical instructions and therapies as prescribed by healthcare professionals. Regularly review and update care plans to ensure they remain up-to-date and relevant.
4. Resident Advocacy: Acts as an advocate for residents, ensuring their rights, dignity, and well-being are respected. Address concerns or complaints raised by residents or their families promptly and effectively, working collaboratively to find appropriate resolutions.
5. Medication Management: Assists with oversight of medication administration and management protocols to ensure accurate and timely dispensing of medications. Familiar with navigating and utilizing electronic medication administration record (e-MAR) systems (ECP). Has the ability to suitably manage and account for medication inventory through monthly cycle fills. Conducts proper documentation records and accounts for managing all residents’ medications.
Required:
- Must have verifiable QMAP
- Must be detail oriented

- 2 years of relevant experience
Job Type: Full-time
Pay: $19.00 - $21.00 per hour
