One to One With Youth Inc. is seeking a Full Time Intensive In-Home(IIH) Team Lead LPC/LPC-A, LCSW/LCSW-A, LMFT/LFT-A or LPA who is licensed in the state of North Carolina to perform comprehensive diagnostic assessments, provide individual therapy, and case management to IIH clients in the community.
**$3,00 Sign-On Bonus and FREE Clinical Supervision.
This position requires exceptional assessment, verbal, interpersonal, and writing skills. Must be willing to be on call and pose strong time management skills. Applicant must have a minimum of 1 year experience with MH/SA population.
IIH Team Lead Roles and Responsibilities:
- Provide individual and family therapy for each youth served by the team.
- Delegate appropriate staff to apply clinical expertise and complete necessary tasks as clinically indicated for each child.
- Titrate utilization management for each child’s treatment episode.
- Provide clinical guidance to the IIH team members to inform weekly clinical intervention throughout treatment trajectory.
- Provide clinical supervision of all members of the team for the provision of this service. An individual supervision plan is required for all IIH team members exclusive of the Team Leader.
- Complete reassessment of child’s clinical needs to guide future treatment, including completion of all necessary referrals and discharge paperwork.
Additionally:
- Conduct monthly Child and Family Team meeting for person-centered planning
- Ensure implementation of person-centered plan and its updates
- Clinically Coordinate with identified medical (for example, primary care and psychiatric) and non-medical (for example, DDS, school, DJJDP) providers, engaging community, and natural supports, and including their input in the person-centered planning process
- Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
- Monitor and document the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan
- Provide psycho-education as indicated in the Person-Centered Plan
- Monitor and evaluate the effectiveness of interventions as evidenced by symptom reduction and progress toward goals identified in the Person-Centered-Plan
- Assist with crisis interventions
- Comply with all current and ongoing training requirements as mandated by current state regulations
Required Skills/Education/Experience/Certification:
- Position requires a Masters Degree in Social Work or a related human field.
- Valid license to practice in state of North Carolina
- Provisional or Full Licensure Status
