About the Role:
The Admissions Coordinator is responsible for ensuring that resident information is accurately gathered before and upon admission to facilitate proper placement. This role involves identifying residents whose needs can be adequately met by the facility. Reporting to the Facility Administrator, the Admissions Coordinator plays a crucial role in maintaining an organized and efficient admission process.
Qualifications:
- High School Diploma or Equivalent required.
- Experience working with people.
- Knowledge of nursing home admission criteria (i.e., financial eligibility, level of care, prescreening).
- Knowledge of nursing home guidelines to provide needed information.
- Ability to work well with co-workers.
- Genuine interest in learning and working with the elderly.
- Patience, tact, good verbal and listening skills.
- Enthusiastic about meeting the residents' needs.
Key Responsibilities:
- Gather pre-admission data to determine the resident level of care, payor sources, and facility capability.
- Complete pre-admission and admission inquiry forms accurately per policy.
- Interact with discharge planners to obtain resident information and coordinate admissions.
- Serve as the first point of contact for prospective residents and families, providing comprehensive information.
- Conduct facility tours, highlighting amenities and programs that meet specific needs.
- Maintain knowledge of Medicare, Medicaid, ProCare, and other insurance guidelines to assist families.
- Coordinate room assignments and complete room readiness checks prior to resident move-in.
- Follow admission, transfer, and discharge policies and procedures, notifying relevant departments.
- Act as a liaison between transferring institutions and the facility, establishing good rapport.
- Participate in marketing activities to maintain acceptable census levels.
- Assist with processing terminations and performance reviews.
- Administer employee health plans and resolve benefits-related problems.
- Provide administrative support to the human resource function.
- Create and maintain all employee files.
- Complete required admission documentation within 24 hours of admission.
- Explain facility philosophy, policies, and procedures to potential and new residents and their representatives.
- Promote positive interactions with residents, residents' representatives, facility staff, and visitors.
Physical Requirements:
- Prolonged periods of standing and walking.
- Ability to lift, push, and pull up to 75 pounds.
