Hackley Community Care (HCC) is a Federally Qualified Health Center that offers integrated health services such as dental, medical, behavioral health, and pharmacy to the residents of Muskegon County. HCC is located along the shores of Lake Michigan and Muskegon Lake where people come to Work, Play and Stay.
Under the direct supervision of the Registered Nurse Supervisor, the RN Care Manager collaborates with facilities, health care team members and patient/families to manage mild to chronic conditions/complex illness and coordinate care for specific patient populations across the continuum and between care settings.

Job Responsibilities:
- Participates and engages their patients in the assessment, planning, monitoring and evaluation of the ongoing care for chronic condition and complex illness management.
- Responsible for reviewing documents, establishing plans of care including patient education and the self-management, coordinating resources for effective, efficient, and appropriate patient care.
- Accountable for ongoing monitoring and evaluation of patient outcomes. Performs duties such as population management and coordination, self-management, education, and oversight of the registries as they apply to those elements.
- Assist patients in optimizing their health status; provide interventions designed to avoid hospitalization and emergency room visits.
Job Qualifications:
- Graduated from an accredited nursing program.
- Current active, in good standing, license to practice as a R.N. in the State of Michigan.
- Possess organizational, interpersonal, and analytical skills.
- Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessments and patient education.
- Maintain CPR/BLS certification.
- Passion to work with the underserved population.
