Job Description
Position Summary:
This member of the multidisciplinary team is responsible for conducting psychiatric assessments; assessing physical health needs; making and supporting appropriate referrals to community physicians and for mental health services; providing management and administration of medications in conjunction with the psychiatrist; providing a range of contracted treatment, rehabilitation, and support services, and sharing shift-management responsibility with other staff. The Residential RN shall be supervised and will work collaboratively under the direction of the Medical Director,
Executive Director of Nursing and Qualified Mental Health Professional (QMHP) to consult and resolve the consumers' emergent nursing and psychiatric needs consistent with nursing and mental health best practices.
Additional responsibilities shall include but are not limited to, facilitating admissions and initial assessments in collaboration with Hawaii State Hospital, community hospitals and
ER departments, ACCESS Line and other Adult Mental Health Division contracted providers and collateral contacts. The RN will work collaboratively with Residential Crisis
Managers and Mental Health Technicians to facilitate and support day to day programming and services in the residential setting. The RN will contribute to completing documentation and progress notes related to consumer progress and activities of daily living, will support the completion of and submit incident reports, sentinel event reports, utilization management authorizations, discharges and bed holds as needed.
Position Qualifications:
- Minimum Education: Graduate from a 2-year ADN or BSN degree program. NCLEX-
RN certification.
- Required Experience: One (1) year of nursing experience. Psychiatric nursing experience preferred.
- Required Certification/Registration: Must be licensed to practice in the State of
Hawaii. Basic Life Support (renew upon its expiration) o CPR / FA Certification o Physical (Pre-employment & Annually) o TB Clearance (2-step - upon hire) Annually
Duties and Responsibilities:
- Provide daily monitoring for an assigned group of clients including coordinating and monitoring the activities of the case management team.
- Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the case management team.
- Provide individual supportive therapy and symptom management, ensuring immediate changes are made in the treatment plans as client’s needs change.
- Educate and support clients' families, and advocate for clients' rights and preferences.
- Conduct comprehensive assessment of psychiatric history, mental status, and diagnosis; physical/dental health; use of drugs or alcohol; education/employment; social development and functioning; activities of daily living; and family structure and relationships

Job Description
- Acts as liaison and consult with community agencies and families to maintain coordination in the treatment process.
- Perform shift management and on-call intervention covering evening hours and serve as a backup to evening and weekend staff.
- Must be on-call for four (4) hours (8pm to midnight) including day & evening hours, 7 days per week (SRSP/TLP)
- Document client progress to maintain a permanent record of client activity according to established methods and procedures.
- Participate in daily staff organizational meetings and treatment planning review meetings.
- Provide medication administration and medical services Registered Nurse
- Under the direction of the team psychiatrist and in collaboration with other registered nurses on the team, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the team psychiatrist; evaluate and chart psychotropic medication effectiveness, complications, and side effects; and arrange for required lab work according to protocol
- Organize and manage the system of getting medications to clients.
- Manage pharmaceuticals and medical supplies.
- Perform job coaching, problem solving, and support on and off the job site.
- Aid with nutritional education, housing placement, money management skills, transportation options.
- Provide support, advocacy, and coordination of social, vocational and activities of daily living needs
Additional Duties
1. Assist with Quality Improvement development and monitoring.
2. Participate in team meetings.
3. Prepare written or electronic reports as requested.
4. Interfaces with CARE Hawaii, Inc. staff
5. Participates in conflict resolution as necessary.
6. Maintain chart filing, ensure charts are properly completed thoroughly and monitor charts to ensure charts are always compliant and audit ready.
