Key Responsibilities:
-
Deliver clinical therapeutic and case management services to adults with substance use disorders and/or psychiatric disabilities.
-
Assist individuals in enhancing their current level of functioning within the community.

Work collaboratively with the primary therapist to contribute to and implement the care plan.

Ensure comprehensive and coordinated care for each individual.

Assist members in applying for benefits and health insurance (SSI, SSDI, Medicaid, or Medicare).

Ensure access to local resources such as psychiatric and medical care, housing, rehabilitation programs, and socialization activities.

Actively engage in outreach to clients on the caseload to maintain their involvement in services.

Communicate regularly with other treatment providers.

Work cooperatively with team members to provide holistic care.

Complete all required documentation in a timely manner according to The Crossroads Center’s standards of practice in Electronic Health Record – CareLogic and/or Methasoft.

Meet weekly, monthly, and quarterly billing quotas as established by the organization.
Qualifications:

Associate degree in psychology, social work, or related field preferred; Master’s degree preferred.

CDCA and Valid Ohio Driver’s License.

Two (2) years’ experience providing services to adults with substance use disorders and/or mental health diagnoses.

Knowledge of the effects of trauma. Proven proficiency with both oral and written communication skills.

Organizational skills and the ability to complete multiple tasks a must.

Strong interpersonal skills and the ability to deal effectively with the public, other team members and elected officials.

A flexible work schedule is required in order to respond to clinical needs and other emergency situations.
Physical Requirements:
While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. (The employee must occasionally lift and/or move up to 15 pounds). Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
