Home Care Coordinator Responsibilities may include:
- Assigning/scheduling Home Health Aides to provide services to patients.
- Coordinates HHA PCA services.
- Update schedule changes in HHA Exchange software.
- Build and maintain relationships with contracts to grow business.
- Places all notes in HHA Exchange and be appropriate and pertinent
- Communicate with insurance companies as needed
- Participate in after-hours, weekend and holiday coverage program
REQUIREMENTS:
- Bilingual Spanish is REQUIRED
- Prior Home Care Coordinator experience is required.
- Knowledge of HHA Exchange is a must.
- Must have a Great Welcoming attitude.
- Highly detail oriented
- Knowledge of MLTC
- Knowledge of CHHA
