Description
Provides case management services to youth and young adults (YYA) who are experiencing literal homelessness or are at imminent risk of becoming homeless, with a prioritization and emphasis on YYA who are part of the LGBTQIA+ community. Case Management will focus on employment and/or education, but may also include finding solutions and supports related to other areas of life where client requires assistance in order to meet their goals. This position provides administrative support by maintaining client files, entering data, and completing required documentation in compliance with contracts.
Selected Duties
- Assist YYA who are experiencing homelessness or at imminent risk of experiencing homelessness to navigate resources and connect to educational and/or employment opportunities.
- Provide client-centered, trauma-informed case management services to diverse system involved YYA and help to coordinate services with other agencies.
- Meet with individual clients to establish a safe, healthy case management relationship based on participation, problem-solving, and mutual respect.
- Analyze client barriers and challenges to employment or education, as well as strengths and skills.
- Create appropriate and valuable education and/or employment plans with clients based on individualized goals and needs. Assesses clients' vocational and/or academic functioning on an ongoing basis utilizing background information and work experiences.
- Support client job readiness skills including but not limited to resume and cover letters, interviewing, time management, and professionalism. Conducts job development and job search activities directed toward positions that are individualized to the interests and uniqueness of client.
- Maintain case records and documentation, including but not limited to service plans, assessments, progress notes, intake reports, and incident reports.
- Understand specific needs of YYA who identify as LGBTQ+. Effectively, respectfully serve LGBTQ+ YYA.
- Plan and host an Education & Career Fair. Design and coordinate and design all details, secure vendors and resource providers in attendance.
Requirements
- Bachelor’s Degree in social or behavior sciences field (or similar)
- Minimum one (1) year experience in Case Management, preferably with youth 18-24
- Technological proficiency
- Adaptable time management capabilities
- Effective communication skills
