Job Summary
Parttime RN to cover a few days Monday through Friday (4-6 hours per day). Mornings preferred, however afternoons, evenings, or a variety will also work. This position is back-up for our other two RNs. Provide proper care for the resident of Cronin Home, Inc. in the areas of medication assistance, dosage box set-up and medical supplies. We are a small nonprofit that helps our struggling folks find a safe, supportive and sober living environment to reestablish their recovery from substance use and addictions. Our nurses provide the guidance our population needs to improve the quality of their life.
Responsibilities and Duties
1) Dispense order and check in the residents’ medications into weekly dosage boxes on a biweekly basis.
2) Document all medications brought in by resident at time of admission and throughout the residents’ stay.
3) Initiate and maintain on a monthly basis all current Medication Administration Records (MAR), store medications, set up weekly dosage box and add dosage boxes to the medication cart.
4) Audit MAR and Controlled Inventory Sheets for staff compliance.
5) Maintain an adequate inventory of the residents’ medications and notify the resident to pick up ordered medication.
6) Educate resident about health care trends.
7) Meet with residents regarding health issues and concerns; provide information about medication management and side effects.
8) Maintain communication with Health Care Providers regarding residents’ medical needs when requested.
9) Discuss upcoming surgical procedures with the resident to make sure their aftercare needs can be met at the Cronin Home.
10) Assist the residents with navigation of health care system when needed.
11) Document health care appointments when provided by the resident
12) Provide referral documentation to accompany the resident to appointments when needed.
13) Audit the resident’s medication compliance on a bi-weekly basis.
14) Resolve and discuss the resident medication noncompliance issues. If unable to resolve involve other Administration Staff.
15) Document any changes in health care status into the resident’s chart.
16) Update Staff to changes and concerns regarding the residents’ health.
17) Make appropriate changes to medication labels and the MAR when the doctors’ orders change.
18) Orientate new staff to medication procedures according to Cronin Home Policy and Procedures.
19) When not at the facility, respond to the resident’s medical concerns or emergencies by phone. If needed return to facility to problem-solve.
20) Inventory and order medical supplies for the staff office.
21) Maintain and check the five First Aid Kits located in various areas of the facility.
22) Provide assistance for minor accidents and surgical wounds on an as needed basis.
23) Discuss medication errors with the resident and staff involved and complete follow up documentation on the Incident Report.
24) Disposes of the resident medication when warranted according to the Cronin Home Medication Policy.
25) Compiles new and current information to update the resource Medication Profile Book.
26) Remove and shred all labels of the Resident’s medication prior to disposal of container.
27) Oversee the shift to shift “Controlled Substance Count” and follow up if necessary.
28) Arrange and monitors CPR and First Aid Training.
29) Discuss complex medical admissions with the other Administrative Staff. 30) Maintain adequate amount of various medical forms.
31) Maintain and file medical forms when documentation is complete.
32) Remove and file excess medical information from charts as needed.
33) Attend all staff meetings and trainings as scheduled; if unable to attend notify Assistant Director.
34) Provide medical presentations and specialized teaching at staff meetings; also reinforce policies and procedures at staff meetings when needed.
35) Oversee development, organization of policy and procedure updates. 36) Observe residents’ behavior and promptly chart and report any unusual behavior to the staff. Call the Police non-emergency or 9-1-1 when deemed necessary.
37) Chart on residents receiving medical attention on a regular basis, following the charting guidelines.
38) Communicate to staff issues and concerns and observations in the daily log as they occur and at the end of each shift.
39) Read and follow Staff memos.
40) Perform all duties in accordance with the Ethics set forth by Cronin Home, Inc. and MARRCH.
41) Enforce all house rules and expectations.
42) Maintains an environment conducive to recovery/respect. Assist residents as needed for their welfare.
43) Leaves facility clean and prepared for the next on-coming staff.
44) Helps and volunteers at all Fundraisers.
45) Follows Employee Handbook guidelines.
