As our Director of Patient Care Services, you will play a pivotal role in ensuring that our agency provides high-quality, patient-centered care while adhering to all California and Medicare requirements. Your responsibilities will include:
- Operations Management: Overseeing day-to-day operations.
- Clinical Leadership: Acting as the DPCS to meet Medicare Conditions of Participation.
- Patient Services Coordination: Managing all direct and indirect patient services.
- Acting Administrator: Assuming the role of the Executive Director/Administrator in their absence.
- Guidance and Counseling: Supporting coordinators and Clinical Supervisors to improve home healthcare services.
- Team Management: Assisting Clinical Supervisors in managing clinical teams and strategic planning.
- Patient Care: Contributing to the assessment, planning, implementation, and evaluation of patient care.
- Financial Oversight: Assisting in budget preparation and administration.
- Operational Analysis: Interpreting operational indicators to manage census, volume, or staffing levels.
- Performance Evaluation: Assisting in clinician performance evaluation and staff management.
- Records Management: Ensuring compliance with local, state, and federal laws.
- Inventory Management: Maintaining supplies and equipment for patient services.
- Orientation and Training: Developing and evaluating orientation and in-service programs.
- Performance Improvement: Supporting organizational performance evaluation programs.
- Quality Assurance: Ensuring the safe and compliant delivery of home health services.
- Policy Development: Administering organization policies and procedures.
- Compliance: Ensuring compliance with all relevant regulations.
- Ad Hoc Duties: Performing other responsibilities as assigned.
- Hospice Services Oversight: Directing all hospice patient services and establishing goals for hospice services.
- Clinical Team Management: Working alongside Clinical Supervisors to manage clinical teams and planning.
- Patient Referrals: Reviewing patient referrals prior to admission.
- Educational Programs: Planning and implementing in-service and continuing education programs.
Qualifications:
- Education: Bachelor's degree in a healthcare-related field.
- Experience: 3 to 5 years of healthcare management experience; previous DPCS experience in Home Health.
- Licensure: Valid California RN license.
- Regulatory Knowledge: Understanding of healthcare regulations, including Medicare and Medicaid.
- Clinical Knowledge: Comprehensive understanding of clinical practices and patient care.
- Leadership Skills: Effective team leadership and management abilities.
- Interpersonal Skills: Strong communication and interpersonal skills.
- Organizational Skills: Excellent organizational abilities for efficient healthcare service management.
- Problem-Solving Skills: Capacity to identify and address patient care and operational issues.
- Compliance and Quality Assurance: Ensuring compliance with regulations and maintaining high-quality patient care standards.
- Computer Proficiency: Knowledge of healthcare management software and EHR systems.
- Professionalism: High standards of professionalism and ethical conduct.
- Home Health Experience: 3 years of management experience within the last 5 years in Home Health care.
- Emotional Intelligence: Compassionate leadership with excellent communication skills.
- Observation Skills: Excellent observation, verbal, and written communication skills.
- Business Management Knowledge: Knowledge of business management, governmental regulations, and ACHC standards.
