Agency is seeking a full time Certified Adult/Child Targeted Case Manager to coordinate efforts to ensure that persons served receive holistic service planning, continuity of care, effective services and on-going advocacy in accordance with Medicaid documentation requirements and Center policy and procedures. Certified Targeted Case Managers coordinate the development of individualized, comprehensive service plans, which allows persons served the opportunity to achieve their goals and optimize their level of independence.
Certified Targeted Case Managers act as advocates for persons served; they link persons served with needed services; monitor persons served progress and provide individualized supports. Targeted Case Management services and activities should be provided in community locations which may include: persons served residences, medical and behavioral health sites, hospitals, community resource sites, shelters, and jails.
Responsibilities:
- Provide case management services to individuals and families in need
- Conduct intakes and assessments to determine client needs and develop appropriate service plans
- Collaborate with clients to set goals and monitor progress towards achieving them
- Coordinate and connect clients with community resources and support services
- Advocate for clients and ensure their rights are protected
- Maintain accurate and up-to-date case records and documentation
- Provide crisis intervention as needed
- Supervise and monitor client compliance with court orders or program requirements
Skills:
- Experience in crisis intervention and de-escalation techniques
- Proficient in motivational interviewing and counseling techniques
- Familiarity with social work principles and practices
- Ability to supervise and provide guidance to clients
- Knowledge of workforce development programs and resources
- Understanding of behavioral health issues and treatment options
- Experience in mentoring individuals or families in need of support
- Familiarity with Section 8 housing programs and eligibility criteria
- Knowledge of childcare resources and options for families
Education/Experience Requirements: Certified Behavioral Health Case Manager (CBHCM) /Targeted Case Manager through Florida Certification Board, and if not certified, must be willing to obtain certification within 10 months of hire, Bachelors degree in the Human Services, Psychology, Social Work or a related human services field is required and one full year full time or equivalent experience in working with children/adults who have a serious mental illness; or a bachelors degree from an accredited university or college is required and three years full-time experience working with children/adults with serious mental illness.
Must have a valid Florida Driver's License and a safe driving record.Jacksonville, Florida and surrounding counties. Must be local to Jacksonville, FL
