The Youth Services Program (Y.S.P) Case Manager is responsible for ensuring that a high level of service is provided to all program participants. She/he will foster a community that provides nonviolent solutions by extensive networking and collaborating within the community and reaching out to youth to connect them to wrap around supports and basic needs resources. The Y.S.P case manager will specifically outreach to males and females ages 18-24 experiencing homelessness, at risk of losing housing or safe space placements by providing a wide array of proactive case management services. She/he will have responsibility for programming and compliance with the Old Colony Y Mission Statement, as well as agency operating and confidentiality policies. She/he will reflect and display the YMCA values of caring, honesty, respect, and responsibility.
Requirements:
- High school diploma or equivalent required, Bachelor's degree preferred
- Minimum of one year in performing client-directed services in a field such as social services, grassroots organizations, community advocacy, or youth intervention
- Familiarity with the community, Brockton residence preferred
- Familiarity with ethnic culture of the target population
- Valid MA driver's license and proof of auto insurance required
