**Job Overview:** We are seeking a compassionate and experienced Hospice Nurse to join our team. As a Hospice Nurse, you will provide high-quality care to patients with terminal illnesses, while also supporting their families. This role requires a strong foundation in hospital medicine, including experience in a Level II Trauma Center. You will work closely with patients, families, and interdisciplinary teams to ensure that patients receive the best possible care and support throughout their journey. **Duties:** - Provide hospice care to patients with terminal illnesses, including managing their symptoms, pain, and other health needs. - Conduct thorough assessments and develop individualized care plans that address the patient's physical, emotional, and spiritual needs. - Collaborate with patients' families, caregivers, and other healthcare professionals to ensure that patients receive comprehensive care. - Provide education and support to patients and their families on end-of-life care, symptom management, and other aspects of hospice care. - Coordinate discharge planning and ensure that patients receive the necessary resources and support after leaving the hospice program. - Utilize ICD-9 coding and other relevant medical codes to document patient care and billing information. - Participate in health coaching and counseling sessions to help patients and their families navigate the healthcare system and make informed decisions about their care. - Maintain accurate and detailed records of patient care, including medication administration, lab results, and other relevant information. - Provide support and resources for patients with dementia, including assisting with daily living activities and managing their behavioral health needs. **Experience:** - A Bachelor's degree in Nursing or a related field is required. - Current licensure as a Registered Nurse (RN) in the state of practice is necessary. - Experience in hospital medicine, including experience in a Level II Trauma Center, is preferred. - Experience in dementia care and health coaching is a plus. - Familiarity with medical office procedures and ICD-9 coding is required. - Experience with discharge planning and coordination is beneficial. - The ability to work independently and as part of an interdisciplinary team is essential. - The ability to communicate effectively with patients, families, and other healthcare professionals is critical. - Certification in hospice and palliative care is preferred but not required.
