JOB PURPOSE:
The Intake Coordinator-Clinician is a dual role where 50% of the time is dedicated to the Intake role and 50% is dedicated to the Clinician role. The Intake Coordinator-Clinician will conduct in-person referral screeners using an electronic health record system to initiate services for eligible clients. The Intake Coordinator - Clinician will work with a multi-disciplinary team for case assignments, consultations and care coordination. The Intake Coordinator – Clinician works closely with the Case Manager to ensure referrals that are denied services receive a warm hand-off to another community partner. The Intake Coordinator - Clinician will work with a multi-disciplinary team for case assignments, consultations and care coordination. The Intake Coordinator – Clinician will provide treatment to Veterans, particularly those who served in the Iraq and Afghanistan wars, active duty members, and their families. The Intake Coordinator - Clinician will have training and experience in trauma and Post Traumatic Stress and will be proficient in providing individual, couples, and family therapy with an emphasis on delivering evidence-based care.
ESSENTIAL JOB RESPONSIBILITIES:
- Conduct initial referral screeners, intakes (as needed) with Veterans and /or family member. Coordinate case assignment with multi-disciplinary team. Document detailed and confidential intake information on the electronic health records (EHR) system; maintain comprehensive client referral tracking system. Prepare reports as requested by Clinical Support Coordinator or Assistant Clinic Director.
- Provide direct treatment services for Veterans and their families. Conduct clinical intake assessments on Veterans and family members as needed. Provide individual, couples, and family psychotherapy with a focus on delivering evidence-based treatments.
- Documents clinical practices for each client in Electronic Health Records including: screenings, biopsychosocial assessments, diagnosis, treatment plans, progress notes, and discharge summaries.
- Collaborates and coordinates with caregivers, physicians, medication management, case management and all other disciplines to provide the best care to clients as needed. Engages in training activities to improve professional services.
- Attend weekly MFC clinical supervision and administrative meetings.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Master’s Degree in Social Work or Counseling with an active license in the state of Texas e.g., LPC or LPC-Associate, LMFT or LMFT-Associate, LCSW or LMSW. Maintain license and/or certifications as required by state regulations. Stay current in the field of mental health, counseling, case management and other related social services.
EXPERIENCE: 3+ years post-graduate experience in intake coordination and/or case management preferred. Experience working with Veteran and Military population highly preferred. Experience required in assessing immediate risk to self or others for the population served. Bilingual candidates are highly preferred.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.
LICENSES: Driver’s License with clear record.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates
