Job Summary: As a Hospice MSW, you will work as part of an interdisciplinary team to provide compassionate care and support to patients facing terminal illnesses and their families. You will assess the psychosocial needs of patients and families, provide counseling and emotional support, and help them navigate the challenges associated with end-of-life care.
Responsibilities:
- Psychosocial Assessment: Conduct comprehensive assessments of patients and families to identify emotional, social, and financial needs.
- Counseling: Provide counseling and supportive interventions to patients and families to help them cope with the emotional impact of terminal illness and impending loss.
- Care Planning: Collaborate with the interdisciplinary team to develop care plans that address the psychosocial needs of patients and families.
- Resource Referral: Connect patients and families with community resources, such as financial assistance, support groups, and bereavement services.
- Advance Directives: Assist patients and families in understanding and completing advance directives and other legal documents related to end-of-life care preferences.
- Education: Educate patients and families about the hospice philosophy, services available, and what to expect during the end-of-life process.
- Crisis Intervention: Provide crisis intervention and support during periods of emotional distress or family conflict.
- Documentation: Maintain accurate and timely documentation of all assessments, interventions, and patient interactions in accordance with organizational policies and regulatory requirements.
- Interdisciplinary Collaboration: Work closely with physicians, nurses, chaplains, and other members of the hospice team to ensure holistic care for patients and families.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited school of social work.
- Licensed or eligible for licensure as a social worker in the state of practice.
- Previous experience in hospice or palliative care preferred.
- Strong interpersonal and communication skills.
- Compassion, empathy, and ability to work with patients and families during a challenging time.
Working Conditions:
- Typically works in hospice facilities, patients' homes, and occasionally in hospitals or nursing homes.
- May require travel within the service area.
- Often involves emotional situations and may require flexibility in working hours.

Pay: $85.00 - $95.00 per hour
