Minimum Education
- Holds an active Registered Nurse license, and is eligible to practice in the state of Kentucky.
- ACLS required
- CPR required

ESSENTIAL FUNCTIONS
- Completes initial, 90 day, and annual assessment of patient.
- Makes ID Team (physician, social worker, dietitian, and director) aware of any issues, concerns or medical needs that were voiced during initial assessment.
- Provide education to patients- infection control, access care (AVF, AVG, or catheter), fluid restriction, process for involuntary discharge, grievance policy, emergency take off procedure, importance of adherence to treatment regimen and complications that occur when treatments are missed.
- Maintains proper documentation for up to date patient vaccinations (flu, pneumococal and hepatitis).
- Develop and implement nursing process.
- Enforce approved policies and procedures.
- Be familiar with all emergency equipment and all emergency operational procedures.
- Performs competent hemodialysis treatments.
- Recognize, interpret, and document signs and symptoms of special care needs.
- Monitor patients for potential problems with hypo tension, inadequate fluid removal, access related problems such as inadequate BFR (blood flow rate), or increased venous pressure and excessive bleeding.
- Maintains proper technique when accessing AVF, AVG, and catheters.
- Demonstrates Dialysis water system monitoring.
- Notifies BioMed if there is a problem with the water system.
- Follows proper procedure if Chlorine/Chloramine levels are elevated - notifies physician, BioMed, and the Director.
- If Chlorine/Chloramine levels are elevated protocol is followed for increased monitoring - every 30 minutes with documentation in the log book and until the physician deems appropriate to return to routine monitoring.
- Mixes acid bath according to physician order.
- Monitors ESRD labs and discusses plan of care with physician.
- Performs set-up and maintenance of hemodialysis equipment.
- Trouble shoot machine alarms as appropriate.
- Safely operates all dialysis related equipment.
- Reports machine problems to biomedical technician.
- Cleans, disinfects, and rinses dialysis machines according to protocol.
- Follows infection control procedures.
- Administers medications and documents accordingly.
- Assist physicians during patient rounds.
- Transcribes and implements physician orders.
- Seeks to improve work processes and participates in work improvement processes and committees.
- Participates in staff training.
- Participates in Performance Improvement activities.
- Participates in Patient Care Planning meetings.
- Participates in gathering data for comparative and trending to aid in identifying opportunities for improvement.
- Responsible for reporting all incidents, information, complaints, or problems to the Director of Dialysis.
- Meets all in-service requirements.
- Monitors lab values if required during antibiotic therapy - notifies physician of lab result.
