Remote Triage Nurse
GENERAL QUALIFICATIONS:
1. Licensed or eligible in Nebraska as Registered Nurse or Licensed Practical Nurse without restrictions.
2. Graduate of an accredited nursing program.
3. Minimum of 4 years’ experience as an RN
4. Prefer minimum 1-2 years’ experience in Urology
5. Ability to maintain confidentiality.
6. Excellent Customer Service Skills
7. Knowledge and ability to use various software packages including Microsoft Office,
E-mail, Electronic Medical Records (EMR), Practice Management and other incidental applications.
8. Excellent organizational skills.
9. Excellent communication skills.
10. Ability to work cooperatively with others to accomplish tasks in a team-based environment.
GENERAL SUMMARY:
The Remote Triage Nurse will play an integral role in communication and coordination of care between the patient and their Servicing Urology Provider. When appropriate, the patients' symptoms will be assessed and triaged in order to ensure the appropriate level of care is followed in a timely manner. The Nurse will utilize our Electronic Health Record software to coordinate and document tasks, take phone calls when needed, as well as coordinate with outside facilities related to their Urological care needs. The Nurse will coordinate care and schedule the patient to be seen when appropriate following our Triage Guidelines and Protocols. The Nurse is to ensure prompt identification of patient conditions considered to be high risk conditions and communicate with their respective Provider as necessary. The Nurse will appropriately delegate tasks to other medical staff based on established protocols and procedures. This position requires an individual comfortable with working remotely and independently but with excellent communication with the patient’s assigned Urology Physician and Advance Practice Provider (APP). This position reports directly to the Clinic Nurse Manager. This position will require orientation in person at The Urology Center. Once the 6-12 week orientation period is successfully completed and the Practice Administrator has signed off, this individual may transition to remote work from their home office. Telecommuting policy will be required to be signed annually for all remote work.

SPECIFIC COMPETENCIES:
· Coordinate and implement patient care plans as established by the Physicians and Advanced Practice Providers (APPs) in a timely manner.
· Communicate the medical care plan as ordered by our Physicians and APP’s along with the necessary education to the patient and their families.
· Utilize digital, verbal and written forms of communication along with TUC approved forms of alternative media such as digital resources to enhance the patient experience and assist the patient in their general understanding of their condition and plan of care.
· Provide nursing triage by collecting appropriate information over the telephone or patient portal in an efficient manner to determine the urgency of a health problem as well as determine whether medical intervention is needed and how soon treatment should begin.
· Communicate directly with patients on the telephone or via patient portal to evaluate, assess, and document patient concerns.
· Assist with EMR data entry including labs, correspondence notes, ancillary results, and other pertinent items in a timely manner in order to implement safe and quality nursing care.
· Assist members of the healthcare team with medication refills and prior authorizations for medications in a timely manner.
· Assist with scheduling patient appointments, procedures, and surgeries as directed by providers and per protocols.
· Accurately document patient information and nursing encounters in the EMR.
· Place orders in the EMR at the direction of the medical providers.
· Provide patient education and advice related to and including pre and post-operative instructions, medication administration, provider established plan of care, follow up appointments, ancillary test results, and when to seek emergency care.
· Utilization of critical thinking, advocacy, and nursing skills to assess patients and ensure they are receiving appropriate clinical points of care within appropriate time frames set forth by pre-established Clinic Appointment Timeframe Guidelines.
· Other duties as assigned by The Office Administrator, Management, or Physicians.

WORK ENVIRONMENT REQUIREMENTS:
1. Employee must be able to provide home internet of 15mb downloading and 2mb uploading speeds without routine disruption.
2. A secure designated home work space free from distraction or noise as well as safe and ergonomically sound. Public space work is not permitted.
3. Not a substitute for illness or continuous childcare
4. The Urology Center will provide equipment for employee’s workstation including and limited to Desktop Computer, Monitor, Phone, and Keyboard that is only to be utilized for business related to employment with The Urology Center P.C. These will remain property of The Urology Center.
5. Must attend 1-2 meetings per month per Manager’s discretion in person at The Urology Center PC, 111 South 90th Street, Omaha, NE 68114
6. Must reside within 45 miles of above address
