Clinical Supervisor (Part-time)
We are currently seeking a qualified individual to join LUCENT HEALTH, Home Health Services team. Someone who has experience, knowledge, and leadership skills, who is dedicated and passionate about their career and the communities we serve, and who is looking for growth and advancement.
Duties:
- The Clinical Supervisor maintains the quality of patient care, in accordance with the Agency’s objectives and policies, through coordination, implementation and evaluation of the Home Health services provided.
Job Responsibilities:
- Review, and investigate, the clinical content, diagnoses, medical history, medications, fee status, and other procedures, from the records of patients considering PDGM.
- Get all patients records ready for OASIS transmittal and POC creation.
- Assist with Pre-Claim Review process.
- Help with identifying, and evaluate, patient needs and appropriateness of home care, by reviewing the initial intake screening form, before assignment of direct care staff.
- Participate in community related functions, as necessary.
- Assist with the orientation of new professional staff, to familiarize them with Agency policies and procedures.
- See patients in the home setting, as needed.
- Perform other duties as directed by the Administrator or Director of Nursing.
- Cover for the Director of Nursing when needed.
Job Conditions
- It is primarily a desk job, which essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication.
- Travel is required, by car or airplane to local, out-of-town, or state seminars, conferences, or meetings.
- The ability to communicate well, both verbally and in writing, in English is required.
- Must have a driver’s license and be willing, and able, to drive to patients’ residences when needed.
- The ability to access patients’ homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
Qualifications
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in the state of Florida, as a Registered Nurse.
- Must have at least two years of homecare experience and, at least, one year of supervisory experience.
- Having knowledge of Medicare guidelines.
- Must have a criminal background check.
- Must have a current CPR certification. Online certification is not accepted.
