Overview:
We are seeking a detail-oriented and knowledgeable MDS Coordinator to join our team. The MDS Coordinator plays a crucial role in ensuring accurate and timely completion of Minimum Data Set assessments for our patients.
Responsibilities:
- Conduct comprehensive assessments of patients' health status
- Collaborate with healthcare providers to ensure accurate documentation of patient care
- Maintain up-to-date and accurate medical records
- Utilize knowledge of medical terminology and coding systems
- Coordinate with interdisciplinary teams to develop individualized care plans
- Ensure compliance with regulatory requirements and standards
- Monitor and update patient assessments as needed
Qualifications:
- Bachelor's degree in Nursing or related field preferred
- Previous experience in health information management or medical documentation
- Strong knowledge of anatomy and physiology
- Familiarity with Epic, Athenahealth, or similar electronic health record systems
- Understanding of home care practices and regulations
- Proficiency in medical terminology and coding
Preferred Skills:
- Experience with MDS assessments
- Certification in MDS coordination
