Program Overview
Central Connection (formerly known as Mental Health Access Point) is seeking a full time Probate Services Director for our program at 311 Albert Sabin Way, Cincinnati, OH 45229.
Duties:
- Oversee housing opportunities for individuals diagnosed with a severe and persistent mental illness.
- Collect clinical information, determine level of care, authorize to treatment facilities and work on a multidisciplinary team to actively coordinate the ongoing care of authorized clients.
- Weekly offsite case staffing with providers and clients.
- Collaborate in a fast paced team centered environment.
- Manage crisis phone calls, provide resource information to community providers including case managers.
- Conduct telephone assessments for Mental Health Probate Court and coordinate Mental Health Probate Court hospital admissions as needed.
Minimum Qualifications:
- Relevant experience in assessing the symptoms and needs of persons with severe and persistent mental illness.
- Knowledge of community mental health services, working knowledge with persons with chemical dependency.
- Hospital or case management experience.
- Basic computer skills and good communication skills.
- Compassionate approach to working with others.
Preferred Qualifications:
- Master's degree in Social Work, Counseling or other related field.
- Ohio licensure in either Social Work or Counseling.
