Seeking a Community Case Manager specialized in working with clients who have sustained an Acquired Brain Injuries.
Community Case Manager would be responsible for completing assessments, addressing complex clinical needs as well as interacting with families experiencing numerous issues and complicated dynamics.
The Community Case Manager will have extensive contact with community-based service providers, establish medical linkages, and access public benefits. Community Case Managers need to demonstrate the ability to work independently, be organized, follow through on recommendations, and have excellent written and verbal communications skills.
Must have reliable transportation to travel and current computer and technology skills.
This position requires traveling to clients homes and working with clients in their community. Flexible Monday through Friday schedule between 8am - 5pm.
Qualifications: Bachelor's Degree in health or human service field with at least one year of Community Case Management experience, or four years' experience working with individuals with disabilities.
