This is an in office and traveling to homes position, not remote from home.
Plans, organizes and delivers agency telehealth service in collaboration with telehealth nurse manager. Coordinates and plans visits with PCN with goal of delivering superior clinical patient outcomes and high level patient satisfaction. Works with Clinical Coordinator group, Chief Clinical Officer/Clinical Director, Clinical Supervisor, Clinical Educator, QI/PI Coordinator and Hospice Director in improving clinical care provided to Agency patients and in the community.
A competent LPN or RN, with the knowledge and competency in the provision and delivery of home care services, and 2 years of clinical experience.
Responsibilities:
- Provides telehealth visits, in coordination with PCN/case manager, in accordance with physician ordered plan of care and in accordance with disease management protocols including initial installation patient teaching visit. Documents visits (same day) in telehealth/EMR systems and implements appropriate and timely follow-up with patient, physician, and other members of agency team, particularly PCN with recommendations for next visits – in home or via telehealth.
- Educates patients on telemedicine program, assuring proper utilization of all equipment – devices and functionality e.g. consistent and regular use of video and other ancillaries, in accordance with patient plan of care.
- Maintains clinical competency through home visiting as requested.
- Ensures coverage of telehealth services during scheduled and unscheduled absences.
- Coordinates patient care with case manager and patient team.
- Participates in the development of clinical protocols of care for common diseases; identifies and implements standardized protocols of patient care for pertinent diagnoses incorporating telehealth technology.
- Educates the community and referral sources on telehealth services.
- Participates in Quality/Performance Improvement activities.
- Other duties as assigned by telehealth nurse manager.
- Coordinate with intake, clinical staff and referral sources for early identification of patients appropriate for telehealth.
