You will be involved with daily rehabilitation in a residential setting to include assisting with cooking, cleaning, laundry, shopping, budgeting and other independent living skills. Duties also include helping plan and implement in-home and community-based activities to meet the needs and desires of the participants, helping them work on their individual goals and documenting their progress.

Responsibilities and Duties:
Essential Job Functions:
· Responsible for the overall care and well-being of participants.
· Formulate and input documentation within an electronic data base.
· Administer medications (training for QMAP Certification will be provided if not already certified).
· Assist participants with daily living skills including laundry, cooking and housekeeping, grooming, etc.
· Help coordinate participant activities.
· Maintain a professional and positive attitude.
· Experience dealing with difficult situations.
Qualifications and Requirements:
· High School Diploma or GED Equivalent is required.
· State of Colorado Qualified Medication Administration Person (QMAP) certification is preferred, but if not held Company will pay for QMAP Training and certification depending on shift and duties.
· Must be at least 18 years of age or older. Applicants under 18 will not be considered.
· Must possess basic cooking skills/knowledge.
· Current CPR/BLS Certification from the American Heart Association or the American Red Cross is preferred, but if not held Company will pay for training and certification.
· Must have written and computer skills. Experience with charting on Electronic Health Record is preferred.
· Must submit to and pass a pre-employment drug/alcohol screening and criminal background check.
Pay: Competitive, $17.00 - $18.50 per hour depending on experience, education and other factors.
Supervises: No direct reports with posted position, although Company provides opportunities for advancement to other positions.
