Summary: The Day Program Provider facilitates life skills training for mentally ill adults; works as part of the multidisciplinary treatment team that addresses both behavioral health and primary care needs. We place a strong emphasis on psychoeducation groups and teaching the life skills that help clients reach their maximum level of self-management and independence.
Minimum experience and education: High School diploma or GED required.
Day Program Provider requirements:
- Plan, develop, conduct, and monitor activities in the Day Treatment and Psychosocial Rehabilitation Programs (both on-site and off-site activities). Transport clients in CHS vehicle as needed for program-related activities.
- Assist clients in developing skills that will enhance their independence and normalize role functioning and facilitate overall health and wellness. Skills domains covered include daily living, household management, personal self-care, community living skills, vocational, educational, primary care health and wellness, behavioral health and wellness, financial, communication and social skills development.
- Teach functional skills to a wide range of individuals; serves as a role model for clients in social interactions and daily problem-solving.
- Assist with the monitoring and management of the client group fund.
- Assess clients for admission to the program, conduct screening interviews, and complete the related documentation. Maintain confidentiality.
