The Health Educator will develop and oversee the education and maintenance of consumers’ health plan in a residential setting. This position will partner with a multi-disciplinary team to optimize the healthcare needs of individuals served who suffer from developmental disabilities, Mental Illness, and traumatic brain injuries. The Health Educator is responsible for meeting the health care needs of consumers in the rehabilitative/habilitative setting, across the life span, and in the face of chronic and acute illness/injury.
Registered Nurse Roles and Responsibilities:
- Guide, and ensure that all needed health care appointments are met for all residents.
- Coordinate health updates and educate external and internal team members, other physicians, and direct care workers on all aspects of the residents changing health care needs.
- Oversee and ensure that all orders and medications are accurate, available, passed and documented correctly.
- Monitor vs, intake & output, medication efficacy, and medication side effects.
- Participate in the on call rotation.
- Provide direct care and treatments to all residents as needed.
- Triage and guide all emergency health situations.
- Complete HCA (Health care appraisals), SOC’s (health care plans), and visit notes on residents.
- Update monthly/quarterly reports/hand offs on all residents.
- Schedule, prepare and execute psych and primary care rounds.
- Attend all outside primary care, psychiatry, and specialty appointments.
- Complete all intramuscular and weekly injections.
- Ensure that all labs are ordered, drawn and on the chart as needed.
- Ensure the medical record is complete and up to date.
RN Qualifications and Education Requirements:
- Registered Nurse (RN) with current Michigan license
- High level of Nursing ethics
- Working knowledge of commonly used medications and treatments
- High level of responsiveness with appropriate use of nursing interventions
- Excellent communication, teaching, and organizational skills
