Position Overview:
As an MDS Coordinator, you will be pivotal in enhancing the well-being and quality of life for our residents. Your role will involve conducting comprehensive MDS assessments, ensuring adherence to state and federal regulations, and optimizing Medicaid and Medicare reimbursement processes.
Key Responsibilities:
- Conduct thorough and timely Minimum Data Set (MDS) assessments to comply with regulatory standards.
- Collaborate with the interdisciplinary team to develop and implement personalized care plans for residents.
- Maintain detailed and accurate records, ensuring compliance with all relevant regulations and guidelines.
- Oversee and enhance Medicaid and Medicare reimbursement processes through precise and meticulous documentation.
Qualifications:
- Current Texas State license as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) from an accredited nursing program.
- At least 1 year of clinical nursing experience, preferably in a long-term care setting.
- Strong communication, organizational, documentation, and time management abilities.
- Proficiency in reading, writing, and speaking English to meet federal and state documentation requirements.
