Role Responsibilities: This position, under the supervision of the Director of Clinical Services, is primarily responsible for intake assessments and determination of eligibility for services of persons requesting assessment for admission into behavioral healthcare services with PTSD Foundation of America. This position is responsible for all documentation of services, including the completion of uniform assessments, progress notes, psychosocial histories, and diagnostics. LPHA – Intake provides assessment and diagnostic services to target population clients.
This position completes eligibility assessments for individuals discharged from a hospital and those reporting from community environments. Intake appointments will be completed in vivo or via telehealth. Additionally, this position is also responsible for supporting programming with urgent crisis assessments/needs on site. LMHP – Intake Routine hours fall between Monday – Friday 7:00 a.m. to 7:00p.m with the flexibility to meet organizational and client needs. This is routinely an 8-hour workday. The position is responsible and accountable for the adherence to and promotion of the compliance policy.
Essential Duties and Responsibilities. Standards of Performance.
Essential Duty 1 Percentage of Job 40%
Completes eligibility assessments.

Assesses clients to determine if mental illness is present and determines eligibility for services.

Completes uniform assessments, psychosocial histories and progress notes.

Diagnoses accurately mental illness/SUD per criteria set forth in DSM V and completes diagnostic form.

Determines eligibility for services in accordance with published guidelines.

Communicates with other agencies to facilitate discharges.
Essential Duty 2 Percentage of Job 20%
Accurately documents assessments and services.

Documents assessment and provides data for continuity of care.

Documents that services provided are authorized within guidelines.

Completes all documents on day services were provided.

Demonstrates level of computer knowledge and skills necessary to utilize E.H.R.
Essential Duty 3 Percentage of Job 30%
Provides support services to both Programming on site.

Assists in co-facelifted groups as needed with assigned Peer staff.

Takes the lead on de-escalation of clients as necessary on campus and completes crisis assessments.

Communicate with other assigned staff members regarding the disposition of any crisis interventions that have taken place.

Communicates regularly with supervisor and works as team member.

Qualifications and Requirements
Education

Master’s degree in psychology or related field.
Experience
Required Experience

3 years of experience in a mental health setting

Veteran (preferred)
License/Certifications

LPC, LCSW, LMFT, Licensed Psychologist

Texas driver’s license in good standing as defined by Texana Center’s procedure.

Compensation: The PTSD Foundation will provide the following compensation:

Salary: $75,000 – 90,000 (dependents upon experience)
