Success Coach job involves implementing and conducting case management, services, and training for participants. Meet regularly (in-person or virtually) with participants to assess program progress, provide supports and constant encouragement. Provide meaning coaching opportunities to help program participants with problem solving skills, life-skills, setting priorities and identifying values.
Essential Duties and Responsibilities:
· Provides career navigation, coaching, and stabilization resources, to facilitate positive outcomes for participants seeking to advance in career opportunities.
· Responsible for delivering holistic and comprehensive strength-based case management.
· Complete a comprehensive strength discovery assessment that provides quality information about each participant’s strengths, family supports, academic challenges, risks, and barriers to success, covering multiple domains, e.g., individual, family, school, and community.
· Provide support services to enhance program participants successes such as community referrals, sending employment opportunities and job retention coaching.
· Conduct individualized goal development, long-term plans, and progress monitoring.
· Meet with each participant weekly or bi-weekly based on needs identified in the assessment.
· Meet with parents/caregivers to establish rapport and then as needed to provide support and connection to services. Be available evenings and weekends to meet with participants and families. As needed.
· Support participant in creating clearly stated academic, training, and personal goals that address risk factors identified during the assessment process which may include review of the results of the PYDI assessment tool.
· Document the progress participant make toward meeting their goals and work with the participant to develop new goals as needed.
· Collaborate with community partners to address challenges, advocate on behalf of participant, and connect to services such as training.
· Monitors and evaluates participant’s progress; provides guidance and coaching to help participants resolve issues and remain motivated to obtain and maintain employment.
· Ensure participant information is timely entered into appropriate form, database or report and maintain confidentiality requirements.
· Comply with agency’s policy and procedures and Commission on Accreditation of Rehabilitation Facilities standards.
· Bachelor’s degree in education, social work, counseling, psychology, or related field preferred or equivalent years of direct experience in case management. Experience may be considered in lieu of education at discretion of the Vice President of Mission Services.
· Minimum 1 year of experience working with participant and families recommended. Additional Experience for Lead Success Coach: Minimum of 2 years’ experience as a case manager. Experience with wrap-around services.
· Experience in using various career exploration tools and assessments.
· Experience in assessing work experience, educational attainment, transferable skills, aptitudes, abilities, needs, interests, readiness for employment, and barriers that hinder employment, highly desired.
· Strong counseling and coaching skills. Excellent interpersonal and written communication skills.
· Computer literate. Excellent organization, communication and interpersonal skills.
· Bilingual (English/Spanish) highly desirable.
