As a Client Care Manager, you would assist clients and their families with decisions related to aging, available services, quality of life, and long-term care planning. In addition, you would work individually and within the team to support our growing business by generating qualified prospects for services. We are looking for entrepreneurial, self-motivated individuals who excel at building new relationships, maintaining relationships, and working in a team environment to provide premium services to our clients.

Essential Duties & Responsibilities (include but are not limited to):
- Uses professional care management skills to independently assess and reassess client status, including physical and mental health, social resources, environmental safety, economic resources, and functional ability.
- Staff and supervise the care team that follows the individual care plan that you create to care for clients. - Calling on and developing new referral accounts in the local health and senior care community. Contacts with local discharge planners, social workers, and physicians are preferred. - Managing ongoing relationships with key accounts, tracking their referral activity, ensuring their satisfaction with our relationship, and conducting all necessary follow-up.
- Drafts, implements and revises the client Plan of Care to ensure appropriateness, effectiveness, efficiency, and client satisfaction. - Collaborates with families and other service providers to facilitate care coordination and information sharing. - Provides supportive counseling and conflict resolution with clients and family members, as an advocate for the well-being of the client. - Makes referrals to appropriate community agencies such as crisis intervention, legal advocacy, and counseling agencies for medical, mental, physical, financial, legal, and social needs.
- Taking after-hours and emergency calls from key accounts and ensuring service delivery.
Qualifications (Required):
· Bachelor’s or master's degree in Gerontology, Psychology, Nursing, Health, or Human Services.
· A minimum of 2 years of experience in geriatrics and/or care management (other professional experience may substitute).
· Some experience in sales and account management (preferably within home care, healthcare, pharmaceuticals, or medical device sales).
· Ability to interface effectively and professionally with client committees, including family, physicians, attorneys, etc.
· Ability to work in a team environment.
· Excellent written and verbal communication skills.
· PC literacy.
· Valid driver’s license and proof of current auto insurance.
Qualifications (Preferred):
· Care Management Certification (CCM or CMC).
