Position Summary: The Memory Care Coordinator is responsible for overseeing the daily operations of the Memory Care unit, ensuring the delivery of high-quality care and services to residents. This role involves developing and implementing individualized care plans, coordinating activities, and working closely with families and staff to create a nurturing and supportive environment.
Key Responsibilities:
- Coordinate and manage care services for individuals with various needs, including dementia.
- Utilize strong organizational skills to ensure efficient delivery of care services.
- Collaborate with healthcare professionals to develop comprehensive care plans.
- Communicate regularly with residents’ families to provide updates and address concerns.
- Ensure compliance with state regulations and company policies regarding memory care.
- Monitor residents’ health and well-being and collaborate with healthcare professionals as needed.
- Maintain accurate and up-to-date records of residents' care and progress.
- Create a safe, positive, and stimulating environment for residents.
- Participate in the hiring and training of Memory Care staff.
- Conduct regular assessments and adjust care plans as necessary.
- Excellent communication skills to interact with clients, families, and other healthcare professionals.
- Demonstrated ability to handle sensitive information with confidentiality and professionalism
- Other duties as assigned.
Qualifications:
- Proven experience in coordinating care services, particularly in areas such as home care or dementia care.
- Bachelor’s degree in nursing, Social Work, Gerontology, or a related field.
- Minimum of 2 years of experience in memory care or a related healthcare setting.
- Knowledge of dementia care practices and interventions.
- Strong leadership and organizational skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively with residents, families, and a multidisciplinary team.
- Compassionate, patient, and dedicated to improving the lives of residents.
- Certification in dementia care or willingness to obtain certification.
- CPR and First Aid certification preferred.
