Amae Health is seeking a Certified Peer Support Specialist to partner with us in providing care for complex adult patients. Please note this position is on site at our North Carolina clinic in Raleigh (27609).
About the Job
Position Summary
The Certified Peer Support Specialist plays a critical role in establishing and sustaining life-changing relationships between Amae and our patients as you help support them through their care journey. This position is an in-person, patient facing role. The Certified Peer Support Specialist will work collaboratively with patients, co-workers and the community. They will advocate on behalf of patients and help them navigate the health and social services systems in Raleigh. The Certified Peer Support Specialist will work closely with patients to address problems and answer questions, gather and provide information and advice, and connect patients to resources and to the community. The Certified Peer Support Specialist will meet patients in the hospital and in the community, as well as at times at their homes. They will initiate, establish and maintain relationships with patients and their family while developing trust and rapport.
Essential duties include:
The Certified Peer Support Specialist will act as a coach and mentor, and help patients to set goals and work toward developing skills. They will share and discuss common experiences with patients. The Certified Peer Support Specialist will help to build a collective sense of community for patients, and help patients to create meaningful lives in their respective community. The Certified Peer Support Specialist will be responsible for planning, organizing, developing, leading and facilitating group activities, including education and awareness building efforts. The Certified Peer Support Specialist will be expected to complete administrative duties, such as sending and receiving email and telephone calls, and completing required documentation. They will also complete any training required for the position. The candidate will communicate and work collaboratively with team members, attend team meetings, and meet with supervisors to discuss performance.
Qualifications
- Candidates should have received certification as a peer support specialist in North Carolina.
- Candidates should have prior experience with mental health and/or addiction issues.
- They should also have knowledge/familiarity of the mental health and social service systems in Raleigh, and an understanding of patient rights
- The candidate should possess a holistic perspective of “health.” Peers will demonstrate the ability to work effectively in a wide range of settings with people from diverse backgrounds, including patients and co-workers
- Candidates should be comfortable working either one-on-one or in group settings, must possess excellent interpersonal skills and be able to adapt to changing situations
- Active involvement in the community and a willingness to collaborate with others
- Candidates should possess excellent communication skills. They should be comfortable with public speaking and facilitation of group workshops or activities
- Candidates should also be comfortable coaching others, and possess negotiation skills
- Candidates should be well-organized and have some experience planning and designing events and activities
- Basic computer skills are required
In addition to meeting the minimum qualification listed above, additional qualifications useful for this position include but are not limited to:
- Experience and/or training in Behavioral Health
- Experience working in a direct service or administrative capacity
