An Embrace Bachelor of Social Work is responsible for helping older adults with two or more chronic conditions navigate and solve for many of the Social Determinants of Health (SDOH) we will discover as we work them over time. Additionally, you will be responsible for administering assessments within your scope of practice such as the PHQ-9. Lastly you will coordinate with our interdisciplinary team and provide input and feedback into care plans as well as help coordinate other community resources we will recommend to our patients. Our goal at Embrace is to help patients learn to take preventive actions that improve their overall healthspan. We work to reduce their chance of a hospital admission and extend their ability to independently age in place. You and the team will be responsible for their health outcomes by changing the behaviors of patients.
Our Personal Prevention Care service is a suite of tech-enabled services that includes chronic care management, prescription adherence and packaging, remote patient monitoring and more. It is based on a personalized care approach with each patient. You will work with our team and help to develop individualized care plans and monthly goals that patients can accomplish with your help. You are expected to utilize excellent social work, clinical and influence skills.
Embrace’s social workers serve patients in their local area. You will establish relationships with other local healthcare providers serving the older adult population and become known as an outstanding care provider and advocate for senior health. You will work closely with the interdisciplinary team and provide updates on the patient’s progress and make recommendations for their care.
You will work from home and manage your schedule to meet the needs of your patients and the company.

Position Requirements:
· Bachelor’s in social work
· Living in and actively licensed in XYZ
· Minimum of 2 years of experience as a social worker
· Ability to work flexible days and times to meet individual patient needs
· Proven experience and competency using computer systems, Microsoft/Google Office products, Electronic Health Records, State Health Information Exchanges, etc.
· Ability to build trusting relationships with patients, physicians, and colleagues
· Willingness to be an advocate for your patients and take the necessary actions to keep them healthy

· Knowledge on how to resolve SDOH issues with the area you work
· Ability to establish relationships and processes with community partners to ensure our patients have the access to all the resources they need
· Provide exceptional care and empathy for your patients

Compensation
This is a 1099 position. A competitive hourly rate will be provided
