Job summary
Primary responsibility of a Community-Based Case Manager is coordinating all aspects of client care, support, and treatment plans to assist clients in preventing recidivism of homelessness by stabilizing housing situations. The Community-Based Case Manager serves as integral member of multi-disciplinary, housing-focused service team
Summary of essential job functions
The Community-Based Case Manager will be responsible for:
- Providing support services to individuals and families that reside in a Homefull Permanent Supportive Housing program;
- Providing support services to individuals and families during their transition from shelter to PSH living;
- Conducting home visits and assessing daily living skills;
- Completing high quality clinical documentation at the time of services, as part of Collaborative Documentation efforts;
- Maintaining client case records according to agency policies and procedures and HMIS policies and procedures;
- Contributing to quality assurance outcomes for Homefull programs including data completeness, housing retention, increasing income, and recidivism;
- Submitting reports/spreadsheets as needed;
- Meeting productivity goals;
- Participating in ongoing education and training to develop professional skills, achieve QMHS status via Homefull training, and maintaining licensure (if applicable);
- Completing CPST documentation and billing spreadsheets following guidelines;
- Maintaining high standard of client care, contributing to an environment where all clients are treated with dignity and respect;
- Maintaining Homefull's code of conduct and follow all confidentiality guidelines;
- Participating in Homefull's activities and contributing to a positive, safe, and healthy work environment;
- Completing other duties as assigned.
Minimum requirements
- Bachelor’s degree required, preferably in social work, counseling, psychology, or related field.
- Experience/knowledge of homelessness and/or marginalized communities preferred.
Abilities required
- Excellent oral and written communication skills as well as computer skills including Word, Excel, and Outlook.
