Position Title: Substance Use Disorder Therapist – Intensive Outpatient (IOP) Evening Part-time
Supervisor: Clinical Supervisor
Qualifications: Master’s Degree in Behavioral Health with active Kentucky License including CSW, LCSW, LPCA, LPCC or LPA and/or Certified by the state of Kentucky as a Certified Alcohol and Drug Counselor (CADC).

Transitions, Inc. is looking for licensed professionals to join our team with opportunities within Intensive Outpatient Services. Transitions offers an environment for individuals to understand and treat the complex nature of Substance Use Disorders as well as co-occurring mental health disorders.
Job Responsibilities:
- Manage a clinical caseload and provide quality of care services to clients including clinical assessments, individualized treatment plans, individual therapy and group therapy.
- Participate in weekly multidisciplinary team meetings with support staff.
- Assure documentation is in compliance with contract regulatory and agency requirements and best clinical practices and complete all clinical documentation within expected timeframes.
- Participate in weekly individual supervision with your Licensed Clinical Supervisor.
- Represent Transitions, Inc. in a positive and professional manner in all community interactions.
- Provide high quality, compassionate guidance in both individual and group counseling sessions.
- Plan, oversee, facilitate and document a client’s treatment.
- Meet with clients individually as needed.
- Create, review, and update individual treatment plans for each client on your caseload.
- Communicate and coordinate with referral sources, as necessary
- Provide crisis intervention to clients as needed.
- Act as a liaison between referral sources and clients.
- Performs other related duties as assigned.
