Job Summary
The RN is responsible for providing specialized nursing care and support to individuals with intellectual and developmental disabilities. The goal is to improve their health and quality of life by addressing their unique healthcare needs and promoting independence. The role involves providing education and training to individuals and their caregivers on self-care skills, health promotion, and safety measures.
Job Responsibilities
Under the supervision of the Senior Director of Nursing and Director of Nursing, the Registered Nurse will:

Travel to and work at various sites in Queens, Brooklyn, or Staten Island.

Visit individuals in hospitals or rehab facilities as needed.

Provide nursing care and support to individuals with developmental disabilities in accordance with HeartShare Policies and OPWDD regulations and guidelines.

Administer medications, treatments, and therapies as prescribed by healthcare providers.

Monitor and assess the health status of individuals, including vital signs, symptoms, and responses to treatment.

Coordinate annual and routine medical services to ensure all healthcare records are up-to-date and compliant with HeartShare Policies and OPWDD regulations.

Conduct audits of medical records and medication inventory.

Supervise AMAP staff in aspects of medication administration and documentation as prescribed by their primary care physician or licensed providers.

Provide all required training to residential staff.

Provide in-service training and oversight in areas such as:

Bowel Movement Monitoring
-
Diabetes Care
-
Skin Integrity and wound care
-
MARS/Med Errors
-
Head Trauma
-
G-tube
-
Epi-pen
-
Medical Equipment Maintenance
-
Any other pertinent medical conditions or issues

Participate in the Program Planning Team process and meetings if needed.

Complete all written reports and documentation to maintain compliance with agency, state, and federal regulations for residential facilities.

Maintain confidentiality on a need-to-know basis regarding healthcare concerns and diagnoses of the individuals served.

Report all incidents, including allegations of abuse and neglect, and serious reportable incidents, to management immediately upon discovery

Advocate appropriately and professionally on behalf of all individuals.

Serve as a role model, abide by agency dress code, and conduct oneself in a professional manner at all times.

Remain sensitive and responsive to cultural differences of individuals and program staff.

Participate in in-service training and on-site workshops for professional growth and development. Attend external trainings and workshops at the direction of Management and the Senior Director of Nursing. Maintain certification in CPR/AED, as required.

Represent HeartShare, both within and outside the organization, in a manner that promotes the Mission, Vision, and Values of the agency. Personally, adhere to all agency policies and procedures.

Undertake any additional tasks and initiatives as assigned by Management, the Senior Director of Nursing, or the Assistant Director of Nursing.

Qualifications

Current license and registration as Registered Nurse approved by the NYS Department of Education.

One year of nursing experience as well as experience working with individuals with intellectual and developmental disabilities highly desirable.

American Heart Association issued BLS Certificate
