Some Job Responsibilities:
- Responsible for planning, implementing and evaluating the health of the individuals within their caseloads.
- Attends medical appointments and schedules follow-up appointments as per doctor’s recommendations, as necessary.
- Ensures all medical appointments and recommendations are followed up on according to best practice, regulations and/or any specific doctors’ orders.
- Dispenses medications and/or administers and oversees treatments as per doctors’ orders.
- Reviews all medications for any adverse effects and changes and consults with medical providers if any concerns/issues arise.
- In a medical emergency obtains the proper emergency help (i.e. police, ambulance, etc.) or renders first aid and follows up with doctor’s visit, if warranted.
- Participates in ITT or any other relevant meetings as needed/requested.
Job Qualifications:
- High School Diploma/GED
- Valid NYS LPN license
- Experience working with individuals with intellectual and/or developmental disabilities preferred
- Valid NYS Driver’s License.
- Must be able to sit, stand, lift 50 lbs.
