Family Support Specialist
The Family Support Specialist (FSS) reports to the Program Coordinator/ Manager/ Director and Clinical Supervisor / Director.

Job Overview
The FSS works in collaboration with adults, children/youth, families, and community-based supports to coordinate in-home, school-related and/or community-based therapeutic activities. The FSS performs tasks like conduct intakes, assessments, safety planning, and develop/monitor plans of care for children and families participating in TeamBuilders’ Family Support Services (FSS) Program. The FSS also acts as the skills development coach teaching clients how to manage everyday tasks to help them gain independence and success in achieving their individual goals. Life skills coaches work one-on-one to help children/youth, adults and caregivers with unique challenges to develop independent living and/or parenting skills. Successful applicants meet the minimum requirements as well as demonstrate key attributes including integrity, self-driven, goal-oriented, and solution-focused. The FSS is a key member of a client’s/child's/family’s care team.
Responsibilities and Duties
- Forms a helping partnership with clients, families, and collateral practitioners including educational professionals to accomplish service plan goals and objectives.
- Links clients/families to appropriate services offered by TeamBuilders or collateral service providers and monitoring of such services.
- Responsible for engaging families and conducting in-home therapeutic skills development activities with adults, children, family members, and other natural supports.
- Recognizes and utilizes "teachable moments" and modeling in helping families during their normal routine.
- Conducts person- and family-centered intakes, screenings, and assessments.
- Develops person- and family-centered plans of care in collaboration with the care team.
Minimum Pre-Employment Requirements and Qualifications
- Bi-lingual/Bi-cultural Spanish and English, preferred;
- HSD/GED + 3yrs experience; or, Associates in human services related field + 2yrs experience; or, Bachelors in human services related field + 1yr experience; or Masters in human services related field + 1yr experience;
- Prior experience must include working with children with severe emotional disturbances (SED);
- Certificate of Completion of Comprehensive Community Support Services (CCSS) training or Provisional approved by the State of New Mexico;
- Criminal Records Clearance (FBI, state/local) by the Department of Health (DOH) and Children, Youth & Families (CYFD) including state/county/local arrest record background check;
- Adult, Child and Infant CPR and First Aid Certification;
- Crisis Prevention Intervention (CPI) Certification;
- Clean driving record; and,
- Current auto insurance coverage.
