Position Description: The Case Manager and Community Worker will be responsible for maintaining an ongoing caseload of students. He / she will work with each student to create an Individualized Development Plan that will serve as their roadmap for goals as they progress through the program. The Case Management and Community Worker will solicit new community partnerships and manage existing partnerships to ensure that the needs of young adults enrolled in the TBNC program are adequately met. He / she will be the face of the agency in the community, attending community events as needed and maintaining strong relationships with community leaders.
Responsibilities
Case Management
· Provide individual and group counseling to participants biweekly and document assessments.
· Work with each student to create an Individual Development Plan (IDP).
· Meet at a minimum two times per month with each student to track progress on their IDP, and contact resources on behalf of student to acquire and coordinate services including but not limited to childcare, documentation, probation officers, housing, and public benefits.
· Provide crisis counseling as needed.
· Conduct outreach and maintain a computerized resource database of community-based service providers.
· Contact families and related support systems as needed, as well as make home visits as needed to provide support to the student’s progress.
· Assist with student attendance and other record keeping of participant progress and needs.
· Track youth participation in each program component (educational, construction, counseling, leadership development and employment readiness and life skills workshops).
· Maintain records of all contacts with student trainees including date, time, issues presented, and actions taken to provide solutions.
· Advocate with human service, health, court, and criminal justice systems to resolve issues facing young people.
· Meet regularly with staff to develop and maintain a consistent approach to supporting and expanding the personal growth and leadership skill of young people.
· Keep service records of contacts with students.
· Oversee mentoring component.
· Develop Life Skills curriculum and teach Life Skills classes.
Community Relations
· Create and maintain relationships with community resources and service providers.
· Manage and maintain strong relationships with community leaders.
· Provide outreach and recruitment at Rikers Island.
· Attend community board meetings and other community events as needed.
· Arrange community service projects and track outcomes.
· Organize and plan service events including coordinating arrangements for trips.
Qualifications
· Bachelor’s Preferred
· Minimum two years case management experience with high-risk youth and young adults.
· Knowledge of human service, health, court, and criminal justice systems’ programs; regulations, and procedures, as well as alternative programs.
· Ability to maintain confidentiality with client information.
· Able to conduct individual assessments.
· Crisis management skills.
· Strong organizational Skills.
· Strong verbal and written communication skills.
· Ability to maintain professional dress, language, and demeanor with student trainees at all times.
· Sense of humor and nonjudgmental attitude.
· Ability to work some evenings and weekends.
· Ability to work as part of a team.
