The therapist is responsible for direct care to clients including: screening, assessing, diagnosing, treatment planning, providing education, referrals, completing documentation, individual therapy, group therapy, and/or case management. Maintains an up to date and professional individual record for each client served. Completes comprehensive assessments and diagnoses using the Diagnostic and Statistical Manual of Mental Disorders. Recommends placement into substance abuse treatment based upon ASAM level of care. Prepares and maintains a confidential, accurate, organized, and updated health record for each assigned client. Complies with documentation requirements as outlined in program policies, protocols, and procedures. Assists with intake documentation as necessary. Assesses client's strengths, abilities, needs, and preferences. Gathers relevant information pertaining to diagnosis, prognosis, and treatment recommendations. Develops, in collaboration with the client, an individualized Treatment Plan to address identified problems and needs associated with their substance use disorder. Must hold a Bachelor's degree. Must have active licensure with the appropriate Ohio Licensing body: LSW, LPC, LCDCIII, LISW, LPCC, LICDC, LISW-S, LICDC-CS
