ESSENTIAL JOB FUNCTIONS
1. Serves as the medical representative for assigned consumers.
2. Submits written 6 month/annual medical reviews with recommendations for future needs for the clients.
3. On a 3 month/ annual basis, reviews the following:
- scheduled appointments for consumers as needed
- relevant documentation of medical visits
- medical notes and consults for each individual
- reconciles consumers’ medications
- consults with CCH social workers/behaviorist as needed
4. Responsible for follow through of all medical needs.
5. Responsible for teaching proper procedures for medication administration, ordering meds, calling pharmacy, etc as needed with CCH Providers
6. Attends medical appointments as necessary.
7. In conjunction with social worker, checks compliance in giving medications
8. In conjunction with the CCH Social Workers, ensures that the CCH Providers are following appropriate guidelines
9. Functions as a medical resource person for the program and provides support to providers in determining approaches to take in relation to the health management of consumers.
10. Functions as the medical liaison with the provider; responsible for communicating as needed with providers to ensure excellence in the management of the consumers’ medical status.
11. Meets with CCH Social Workers, RN Supervisor, Behaviorist & CCH Director on a weekly basis at CCH case reviews.
12. Reports, in a timely manner, any out of the ordinary medical problems, to the CCH RN Consultant Supervisor / CCH Director.
13. Maintains confidentiality inside and outside the agency in all aspects of clients’ needs/problems and in regards to any issues that arise in any of the providers’ homes.
14. Maintains confidentiality inside and outside the agency regarding any issues dealing with another employee/provider of CRI.
- Communicates to the CCH RN Consultant Supervisor / CCH Director any problems or concerns with another employee/provider while maintaining confidentiality for all persons involved.
16. Will attend DDS nurses meetings as necessary and complete other duties/ as may be assigned

OTHER REQUIREMENTS*
- Possesses a valid CT driver’s license and a good driving record.
- Possesses excellent organizational skills.
- Possesses excellent written and oral communication skills.
- Possesses competent computer skills.

EDUCATION*
Must be a graduate of an accredited Registered Nursing program and possess a current RN license. License will be renewed independently.
