Purpose for the Position:
To assist patients and their families in addressing social, emotional and economics stressors associated with illness or injury.

Essential Responsibilities:
- Assists in the coordination of care in close collaboration with physicians and other healthcare personnel in patient evaluation and treatment to further their understanding or significant social and emotional factors underlying patient=s health problem.
- Assists and educates patient and family through individual and group conferences to assist in developing their plan of care.
- Assess patient level of functioning prior to hospitalization and other hospitalization in developing discharge plan.
- Utilizes community resources to assist patient to optimum social and health adjustment within patient=s capacity.
- Completes social work screenings within 24 hours of admission.
- Documents social work and discharge planning activities into patient=s charts and case records.
- Attends department meetings and continuing education requirements.
- Participants in weekly multi disciplinary discharge planning meetings.
- Knowledge development stages of infancy through geriatrics.
- Knowledgeable of DSM-IV criteria.
To do this job, you must have the following licenses or certifications before being hired:
CURRENT STATE SOCIAL WORK LICENSE
OR BE LICENSE ELIGIBLE.
BACHELOR DEGREE IN SOCIAL WORK FROM AN ACCREDITED PROGRAM
(M.S.W. PREFERRED)
MINIMUM 2 TO 3 YEARS SOCIAL WORK EXPERIENCE
PREFER EXPERIENCE BE IN A HOSPITAL SETTING
