Alternative Home Health Care is a home health agency owned and operated by nurses with 20 years experience in healthcare with the last 10 years specializing in home care management. We are looking for a Physical Therapist in the Northshore area. Responsibilities include:
- Provides services in accordance with organizations accepted standards of practice.
- Admits patient to home care services as needed. Completes initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.
- Develops and revises the patient specific plan of care in consultation with the physician, patient, patient’s representative (if any)and other care team members, based on individual needs of the patient and including patient specific measurable outcomes and goals
- Develops the patient specific plan of care that includes emergency/disaster planning, planning for avoiding emergent care, hospitalizations and re-hospitalizations, and planning for discharge.
- Identifies barriers to desired outcomes.
- Establishes realistic measurable goals with the patient.
- Provides therapeutic treatments.
- Teaches the client/caregiver. Provides patient, caregiver, and family counseling and education.
- Evaluates equipment needs and coordinates implementation to increase functional level.
- Prepares clinical and progress notes that are legible, accurate, and complete and submits them in a timely manner according to agency policy.
- Coordinates services in consultation with the home health staff. Communicates changes in the patient care plan to the physician.
- Coordinates care delivery to meet the patient’s needs, and involves the patient, representative (if any) and caregiver (s) as appropriate, in the coordination of care activities.
- Ensures that each patient, and his or her caregiver(s) where applicable, receive ongoing education and training provided by the Agency, as appropriate, regarding the care and services identified in the plan of care. Provides training, as necessary, to ensure a timely discharge.
- Evaluates and documents the effectiveness and outcomes of care.
- Supervises Physical Therapy Assistants and Para-professionals.
- Participates in in-service programs.
- Plans with the client discharge from services.
- Observes infection control practices according to organization policy and procedure.
- Participates in the Agency’s Quality Assessment/Performance Improvement program
- Performs other duties as assigned
