SUPERVISOR: Nurse Manager
HOURS: M-F 8am-8pm, Some Sat 9-12, Flexible (Evening/Weekend as needed)
CLASSIFICATION: Hourly/Non-Exempt TRAVEL / LOCATION: Flexible
JOB SUMMARY: Working with a diverse patient population, this position will provide relevant nursing support services in a primary care setting to patients with acute and chronic illnesses. Clinical care delivery must meet or exceed goals and expectations for standards of practice and organizational guidelines.
QUALIFICATIONS:

Graduation from an accredited nursing program with a current NYS state licensure in an LPN

Direct experience in primary care preferred (but not required)

Reliable transportation between sites required.

Current Basic Life Support certification (BLS)

Experience with Medent EMR preferred, but not required
PATIENT INTAKE
-
Take/record measurements such as blood pressure, temperature, heart rate, urine dip.
-
Obtain patient’s complaint – reason for the visit.
-
Ask basic screening questions – review medications and allergies, family and social history.
PATIENT CARE
-
Prepare patients for examinations, testing, or treatments.
-
Complete all patient charting and ensure all records are current/complete with accuracy.
-
Provide direct care as assigned/qualified to perform.
-
Perform medical treatments (medication and vaccine administration) as prescribed by a physician, nurse practitioner, physician assistant or midwife.
ADMINISTER TESTS
-
Finger stick hemoglobin, lead and glucose.
-
Pulse oximetry.
-
Perform EKG and spirometry.
-
Collect samples (blood, urine, stool, or sputum) from patients and perform routine laboratory tests.
POST-VISIT
-
Debrief patient for understanding of plan and instructions, provide counseling and help to coordinate care.

Review provider note and orders.
-
ER/Hospital: Check and obtain discharge summary.
TEAM
-
Work collaboratively with clinician (MD, NP, PA, CNM, DO) to provide excellent clinical care. To this end, LPN’s typically perform the following responsibilities under the direction of an RN or other qualified practitioner:

Perform daily Morning Huddle with assigned medical provider per protocol.

For each patient, review DM/HM for needed preventive care, discuss care that is due with the patient.

ER/Hospital Follow up: Check Medent for Discharge summary, if not present, obtain it from Info clique (Kaleida) or HealtheLink, and other resources as they become available.
-
Kids; Check NYSIIS, get shot record ready.
-
Newborns: review newborn discharge summary and/or NYSIIS for newborn Hepatitis b vaccine and enter into Medent.

Obtain Patient’s Chief complaint; What is the reason for the visit?
-
Review meds and allergies. Hit update button. Notify provider if meds are not correct. (ok to add OTC meds) (each visit)

Update Family and Social History.
-
Discuss DM/HM recommendations with patients.
-
Vaccinations – offer flu, ATD (if didn’t have one in 10 years), updated pneumo, Zostavax per guidelines and DM/HM alert

Mammogram
-
Colonoscopy
-
Diabetics – have patients remove shoes and do foot exam, document in Medent under the diabetic foot exam button.

Asthmatic/COPD – get peak flows.
-
For patients who are visibly short of breath, obtain pulse oximetry.
-
Have PT sign release of records for colonoscopy results/pap smears/mammograms if needed (perform elsewhere and not on our chart).

Start prenatal chart notes.

Calculate dating

Putting weight, BP, urine dip directly in chart

Post-visit – debrief the patient and make sure there is clear understanding of ‘Assessment and Plan’ by providing the needed counseling and helping to coordinate care. This involves reviewing the provider’s notes and orders. Go over instructions such as new medications to take, inhalers, glucometer use, and any instructions. Ensure patient verbalizes understanding.

Provide Hospital and ER follow up –

At direction of provider, call patients recently discharged from hospitalization and assist in transition of care.

Phlebotomy
-
Perform fingerstick hemoglobin, lead, and glucose.

Administer nebulized medications. Check Peak flow after administration is complete.

Administer IM Medication as ordered (Toradol, B12, Phenergan, Ceftriaxone, etc.)
PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking and climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation.
TRAVEL REQUIREMENTS: Must be able to travel to and between sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
