Position Summary:
Provide case management, outreach and direct support to foster housing stability and the overall well-being of clients accessing food assistance through Project SOUP. Intensively work with individuals and families on a regular basis to facilitate linkages to a range of services and resources. Provide support and coaching around food insecurity, homelessness prevention, housing search, health and wellness system navigation and other needs of clients relating to their housing and food stability. Collect and track data of clients served.
Essential Functions, Duties and Responsibilities
- Conduct comprehensive client assessment and intake process
- Meet with individuals and families at Project SOUP (or at their homes if homebound) on a regular basis
- Provide crisis intervention, informal counseling support and advocacy services to promote health and safety
- Assist with budgeting and credit repair needs; benefit attainment; access to food assistance, resources for eviction prevention, access to healthcare; addressing court related issues; connections to education, employment, and volunteer opportunities; referrals to mainstream services; and activities related to family, social and community reintegration
- Organize and maintain accurate and up-to-date paperwork, including client files and program reports
- Enter comprehensive individual meeting notes and assessments into online database systems (HMIS)
- Serve as intermediary and coordinator within each client’s network of service providers
- Perform outreach to community partners to learn about resources available to clients, particularly regarding SNAP, WIC, and other food programs
- Attend individual supervision, staff meetings, and community-based meetings
- Collaborate with the SHC Street Outreach team to ensure clients are receiving effective services
- Perform other duties to ensure overall smooth operation of all agency programs

- Demonstrate strong and clear communication both written and verbal.

Education, Abilities, Competencies and Experience Required
- BA in Human Services preferred
- Car and valid driver’s license preferred
- Minimum one year relevant experience with homeless, mental health, and substance abuse services and resources
- Proficient computer and technology skills including, Microsoft Word and Excel and other web-based applications
- Flexible, reliable, motivated and possess positive attitude
- Excellent time management skills and able to meet deadlines
- Organized, able to multi-task and follow-through with tasks to completion
- Work well independently and as part of a team environment
- Spanish language skills required
- Haitian Creole/ Portuguese preferred
Supervisory Responsibility
This position has no supervisory responsibilities.
Work Environment
This position is primarily off base and will require field work under all-weather conditions.
Physical Demands
- Ability to walk up to three miles in a city setting, rural areas, and in forestry.
- Ability to carry a backpack with supplies weighing up to 20 lbs. with supplies needed.
Position Type and Expected Hours of Work
Designated days and schedule will be primarily Monday-Friday though will also be based on the needs of the operation and business, may require weekends, nights and early mornings.
Travel
Position requires minimal travel; based on the needs of the operations and business.
