Job Purpose/Summary
The Mobile Medication Service is a voluntary service provided for adult consumers with serious mental illness who experience difficulty making progress toward recovery. The Mobile Medication Service is designed to create a continuum of care related to medication administration, as well as assessing, monitoring, and education that span the system of care for that consumer.
The Certified Peer Support – Mobile Medication PT/PRN's function on the team is to uniquely engage the consumers, assess their needs/barriers to care, and to reinforce what has been learned and the generalization and application of that knowledge. The position works to educate, support, empower, and engage with the consumers in service to promote quality, holistic care to ensure healthy outcomes, wellness attainment, and ongoing tenure in the community setting. The scope of responsibilities of the team include counseling, crisis intervention, promotion of self-care skills, monitoring of medical, physical, and mental health status/treatments, and coordination of services.
Minimum Qualifications
Education: High school diploma or GED is required.
Experience: Experience as a consumer in the mental health system and with recovery principles and practices.
Licenses, Certifications & Clearances:
· Certified Peer Specialist (CPS) License
· Act 33/34 Clearances
· FBI Clearance
· Valid State of PA Class 1 Drivers License
· Access to properly insured motor vehicle for client transportation and fieldwork and a safe driving record

· Supports and educates consumers.

· Maintains a helping role and intervenes appropriately to meet service goals.
· Completes documentation of service provision as per program requirements.
· Supports consumers as active participants in treatment planning.
· Aids to decrease the utilization of crisis and inpatient services due to medication management issues.
· Assists to ensure success in a least restrictive living arrangement and level of treatment care.
