Primary Care Access Coordinator

Under the general supervision of the Bridge 2 Access Manager, the Primary Care Access Coordinator provides patient outreach in a manner that is culturally sensitive and non-judgmental, schedules appointments, facilitates access and engagement in primary care for patients in our Medicaid ACO (Accountable Care Organization).
- Provides patient outreach, schedules patient appointments, and facilitates engagement in care.
- Supports patient’s ability to utilize digital platform to access virtual visits.
- Documents patient outreach and planning in the patient’s record.
- Identifies barriers to attending medical appointments and resources such as transportation, to address these services when necessary.
- Facilitates completion of patient questionnaires and pre-visit forms.
- Learns and become proficient in available electronic health information systems including, but not limited to, EPIC and EPIC related tools, registries, and Patient Financial Services IT tools
- Manage confidential patient information.
- Triages and manages more complex telephone calls, utilizing courteous customer service skills.
- Maintains confidentiality and privacy, which is consistent with HIPAA (Health Insurance Portability and Accountability) guidelines.
- Performs other related tasks which would facilitate patient engagement in primary care.
- Works as an effective, collegial member of the care team
- Provides support for panel clean-up efforts and other administrative tasks supporting primary care access.
- Follows health plan processes to ensure Medicaid ACO patients whose care is transferred to a new PCP are correctly enrolled and assigned to that PCP.
- Works on special projects as directed.
Working Conditions:
Hybrid position, with in-person requirements based on Clinic schedule.
8 hours per day between 8:00 AM and 8:00 PM, Monday through Friday, as program needs evolve
Periodic travel to ambulatory and hospital-based practices may be required.
QUALIFICATIONS:
- Excellent and effective interpersonal and communication skills.
- Demonstrated ability to work effectively and courteously with various groups of patients, staff, and providers.
- Demonstrated ability to problem solve and function as a resource to other members of the team and resolve complex issues on behalf of the providers and the patients.
- Knowledge of Epic strongly preferred.
- Exceptional organizational skills, flexibility to manage multiple tasks, attention to detail, and accuracy.
- Ability to work independently or within a team environment.
- Demonstrated in-depth understanding of pertinent insurance/medical coverage.
- Demonstrated knowledge of HIPAA Confidentiality and Privacy Policies.
- Highly organized, proactive, and attentive to details.
- Spanish fluency.
