Cornerstone Montgomery – Peer Support Specialist Assertive Community Treatment (ACT)

The Assertive Community Treatment (ACT) team is an interdisciplinary team that works together with consumers in their own environment (home, community, employment, shelter, etc) in an assertive and high-intensity level. The peer specialist is a key member in supporting consumers in their recovery. This position works directly with clients with severe and persistent mental illness and/or co-occurring substance use disorders by helping them identify their goals for recovery and assisting them in meeting those goals. The peer specialist works collaboratively with the other ACT Team staff to assist clients meeting COMAR and fidelity regulations for ACT.
Responsibilities of the Peer Support Specialist with the ACT Team
- Provide client-centered case management & psychiatric rehabilitation services to clients of the ACT team
- Act as a positive role model for individuals
- Provide life skills training, behavior management, health management, crisis intervention, and support to clients to take medication as prescribed
- Maintain a caseload, attend client meetings on time & prepared, and complete all documentation in a timely fashion
Qualifications of the Peer Support Specialist with the ACT Team
- Associates Degree-Bachelor’s Degree preferred
- 1-2 years experience working with adults diagnosed with mental health and/or co-occurring substance use disorders preferred. Personal History as Consumer of Behavioral Health Services.
- Possession of a valid driver’s license and satisfactory driving record (as determined by HR) required. Must have access to a personal vehicle and be willing to use it for work purposes. Maybe required to transport clients in personal vehicle.
- Certificate of completion from Maryland State approved peer support service training program preferred.
