Salary: $70,000 - $80,000 per year

Setting: Exciting opportunity to work with an incredible organization providing community-based health and wellness care in New York City. Join the Governor-created Safe Options Support (SOS) team, comprising a Team Leader, Licensed Mental Health Clinicians, Care Managers, Peer Specialists, and a Registered Nurse.
Position: This is a highly rewarding position focused on stabilizing individuals in the community who are experiencing adverse events, including homelessness. As a member of the SOS team, the Clinician will be actively involved in community outreach, coordinating participants' needs before and after their transition from the streets to homes. This role also involves enhancing their daily living skills, providing supportive counseling, and advocating on their behalf to access community programs.
Daily Life: Your schedule typically runs from 9 AM to 5 PM M-F with some flexibility. Depending on candidate preference, you could work shifts from 7 AM to 3 PM or 1 PM to 9 PM.

Requirements:
- Preferred licensure as LMHC, LMFT, or LCSW, licensed to practice in New York.
- Master's Degree in Social Work or a related field.
- Previous experience in community health and outreach is preferred.
- Strong understanding of community agencies for referral.
- Demonstrate knowledge, skills, and abilities in mental health crisis intervention.
Responsibilities:
- Engage in persistent and assertive outreach and engagement using strength-based approaches, beginning either at known "hang-outs" within the transit system or during an inpatient hospital admission or emergency department visit.
- Collaborate with street outreach teams, local police precincts, local hospitals, the MTA, the Department of Homeless Services, and family members/caregivers to identify those in most need of outreach and care.
- Continuously assess the health and social needs of participants through conversational and observational assessments, and formalized risk assessment tools for high-risk individuals.
- Work in collaboration with the centralized SOR Hub to identify available housing and support participants through the housing process.
- Participate in hospital discharge planning meetings to identify the best community resources for returning patients.
- Provide short-term therapeutic counseling and support to participants pre- and post-housing.
- Supervise case managers and peers on the SOS team.
- Collect and report data as required and use data to inform future care delivery.
- Work with participants and their housing providers to resolve clinical issues affecting their ability to manage and retain supportive housing.
- Foster relationships with community providers to ensure recipients are connected with appropriate services as they transition back into the community.
