#### Role Overview:
As a Hospice Community Liaison, you will play a crucial role in connecting our organization with the community. You will educate healthcare professionals, community members, and potential patients about our hospice services, ensuring that those in need are aware of the compassionate care we offer. Your primary responsibilities will include:
- **Community Outreach:** Develop and maintain relationships with healthcare providers, community organizations, and other key stakeholders in the LA and OC areas to promote hospice awareness and services.
- **Education and Advocacy:** Provide informative presentations and educational materials to healthcare professionals, patients, and families about hospice care options and benefits.
- **Referral Management:** Facilitate seamless communication between healthcare providers and our hospice team to ensure timely and appropriate patient referrals.
- **Event Coordination:** Plan and participate in community events, health fairs, and educational seminars to raise awareness about hospice care and build strong community connections.
- **Patient Support:** Serve as a compassionate point of contact for patients and families, offering guidance and support throughout their hospice journey.
#### Qualifications:
- **Experience:** Minimum of 1 year of experience in hospice marketing, with a proven track record of establishing and maintaining relationships within the hospice or healthcare sector.
- **Education:** Bachelor’s degree in healthcare, marketing, communications, or a related field preferred.
- **Skills:** Excellent communication and interpersonal skills, strong organizational and time-management abilities.
- **Attributes:** Empathy and compassion for patients and their families, ability to work independently and as part of a team.
- **Knowledge:** Deep understanding of hospice care and end-of-life services.
