Summary/Objective
The PACE driver is responsible for the direct transportation of PACE participants to and from their destinations, including home, the PACE Center, medical providers, etc. Position requires constant interaction with PACE participants, caregivers, community agencies, medical providers and PACE program staff and leadership.
This is a part-time position working 1 - 2 days per week.
Essential Duties/Responsibilities
- Transport PACE participants to varied destinations timely, safely, and door-to-door
- Confirm that the PACE transportation vehicles are in proper working order, clean and safe. Update and keep all transportation maintenance records
- Assist passengers on and off the vehicle in a safe and courteous manner. Demonstrate understanding of handling medical emergencies as well as the ability to work with various types of medical equipment
- Follow established schedules and routes, with the flexibility to meet unanticipated changes, determine the safest and most efficient pick- up and delivery routes
- Participate as a member of the interdisciplinary team. Takes part in participant care planning and contribute to notations in the medical record. Report information regarding changes, progress, or unusual events concerning PACE participants to IDT members
- Participate in the maintenance of a safe, clean and healthful day center environment.
- Assist with participant care within their scope at the PACE Center when not transporting participants.
- Assist in delivering vehicles to repair shops as needed.
- Demonstrate professionalism by maintaining a neat and clean appearance, the willingness to learn new concepts, accepting accountability for job performance, the ability to manage time effectively, and promoting compassion and understanding for participants’ needs and wants
- Demonstrate good attendance and punctuality
- Facilitate communication via email, phone and face-to-face meetings with key staff to maintain project flow/contracted timeframes
Required Education and Experience
· High school diploma or general education degree (GED) required
· Must be at least 21 years of age or older
· At least one year experience in transporting individuals with disabilities and working with frail elders
